Amended Tax Return

Submit a copy of the 2018 1040X form.

Child Support Paid Verification

Submit a signed statement of the child support that was paid out of your household in the 2018 calendar year.

DD214 Form

Submit a copy of your DD-214 form to verify your Veteran status

Default Cleared on a Student Loan

We have received information that you have defaulted on a student loan. You are not eligible for financial aid until you resolve this default. Login to www.nslds.ed.gov to view your defaulted loan and contact your loan servicer to resolve the default. Provide documentation from your lender when you have cleared your default. 

FAFSA Incomplete

Your FAFSA contains information that is either missing or requires resolution. Review your Student Aid Report to see what needs updating. Then, correct your FAFSA electronically.

Possible conflicts include but are not limited to:

  • Missing student or parent signature
  • Asset information is missing
  • You are a male under the age of 26 and have not registered with Selective Service
  • AGI and taxes paid were reported as the same amount
  • You or your parent’s name, SSN and/or date of birth do not match 

Identity & Education Statement

Must be completed and signed at the Financial Aid Office in the presence of a Financial Aid Administrator. Please come to the office with a government issued photo ID to complete this form.

Legal Documentation of Date and Birth

The Federal Processor could not confirm your name, Social Security Number and Date of Birth match. Provide your birth certificate OR a copy of your Social Security Card and driver’s license or state issued photo ID. 

Orphan or Ward of the Court Document

Submit documentation regarding your status as an orphan or ward of the court. Documentation of ward of the court must show that you were under state custody until your 19th birthday.

Parent Marital Status Form

The marital status that your parent(s) reported on your FAFSA may conflict with their reported tax filing status. Please complete the corresponding form.

Verify Prior Bachelor’s Degree

You indicated on your FAFSA that you have already received a Bachelor's Degree. Submit a written statement verifying whether or not you have previously received a bachelor's degree.

Selective Service Registration

Submit documentation that you have registered with selective service

Student Marital Status Form

The marital status that you reported on your FAFSA may conflict with your reported tax filing status. Please complete the corresponding form.

Untaxed Income Verification Form

Additional information is needed, please complete corresponding form

U.S. Citizen or Eligible Non-Citizen Documentation

Submit documentation that you are a U.S. citizen or an eligible non-citizen. Typical documents include a passport or a copy of both sides of your green card.

Verification of State Residence

The state and date of you (and your parents if dependent) legal residence needs clarification

Verify Emancipation

You indicated on your FAFSA that you are or were an emancipated minor. Provide documentation that you have been legally emancipated.
