October 15, 2020 - COVID-19 Instructions for All Faculty, Instructors and Staff Responding to Recent Known COVID-19 Exposures or Positive COVID-19 on Campus

In order to protect all members of the QCC community and to help prevent inadvertent asymptomatic spread of coronavirus and mitigate the spread and/or reemergence of COVID-19, this protocol should be followed if a student or employee discloses that they have had a confirmed positive COVID-19 test or have had a recent known positive exposure and has been on campus within the past 14 days.

Action Plan

  1. Instruct the student or employee not to come to campus until a contact tracer has followed up with them.
  2. Instruct the student or employee to immediately complete and submit the COVID-19 Exposure Health and Safety Intake Form
  3. Faculty or Supervisor must send an email to covid19 [at] as soon as they were made aware of a disclosure and include the following:
    1.  Student or Employee name and student ID#
    2. Student or Employee QCC email
    3. Student or Employee phone number
    4. Date of disclosure
