Fallon Health names QCC Alumna Lisa DeWitt
Site Director of Summit ElderCare Charlton
Worcester, Mass., August 14, 2017— Fallon Health, a nationally recognized, not-for-profit health care services organization, is pleased to announce the recent hire of Lisa DeWitt as Site Director of its Summit ElderCare location in Charlton.
In her role as Site Director, DeWitt will oversee the daily operations of Fallon’s Summit ElderCare program—a Program of All-inclusive Care for the Elderly (PACE)— while interacting with participants and caregivers.
DeWitt will lead an interdisciplinary team that includes a range of professionals who specialize in geriatric care, including primary care physicians, nurses, nutritionists, transportation coordinators, home care coordinators, social workers, and rehabilitation therapists, ensuring that each participant’s personalized care plan allows him or her to live as independently as possible.
In 10 to 12 months, DeWitt will also lead Summit ElderCare Charlton as it moves 12 miles to the east, to a soon-to-be-built facility at 108 Thompson Road, Webster, pending final approval from the Centers for Medicare & Medicaid Services (CMS).
Prior to her role as Site Director, DeWitt served as Clinical Nurse Manager and Homecare Nurse Manager at Summit ElderCare.
DeWitt received an associate degree in nursing from Quinsigamond Community College, a bachelor of science degree from Worcester State College and a certificate in legal nurse consulting from Bay Path Community College.
DeWitt currently lives in Dudley, MA.
Fallon’s Summit ElderCare program has six sites in Massachusetts. In addition to Charlton, the sites are located in Leominster, Lowell, Springfield and Worcester (two). Fallon also operates a PACE program, Fallon Health Weinberg-PACE, in western New York. Fallon Health is the largest in provider of PACE in New England and the fifth largest in the country.
